Post-acute care is increasingly important in today’s healthcare environment with a growing number of patients requiring care post hospital discharge. Research demonstrates that the majority of variability in healthcare quality and spending resides with post-acute providers. This is significant for hospitals that are increasingly accountable for the provision of patient care for 30-90 days following discharge via value-based models such as bundled payments. Post-acute care is also impactful for accountable care organizations and clinically integrated networks.

Many healthcare systems are employing a Continuing Care Network strategy designed to lead to improved quality, enhanced patient experience and simultaneous reduction in expenditures. A Continuing Care Network is a selective group of high performing post-acute care providers designated through an objective process. These providers commit to collaborating closely with hospitals and other healthcare stakeholders to advance a series of shared objectives focused on quality, value and patient experience.
